Equipment Request Form

This form does NOT guarantee you the equipment requested, but is intended to simplify the requesting process - if there is a conflict with your request, we will contact you asap. All appropriate certifications must be met before equipment can be checked out from CAT TV.

You may pick your requested equipment up between 3pm and 8pm on our normal business days. Please note, all gear is due back to CAT by 7pm on the requested end-day.

Please limit your requested time period to 1 week.
