The following warranties and representations are made by me in submitting this program for cablecast on Columbia Access TV. I accept full responsibility for the Program's content, and have obtained all approvals, clearances, licenses, etc. which are needed. I have read CAT's Policies & Procedures, and agree to abide by them. I agree to indemnify and hold harmless Columbia Access TV, its personnel and Stephens College from any and all liability arising from or in connection with the Program, or my acts or statements. I hereby grant Columbia Access TV permission to transmit the Program, retain a copy and retransmit the Program at its sole discretion.
