I accept full responsibility for the Program's content, and have obtained all approvals, clearances, licenses, etc. which are needed. I have read CAT's Policies & Procedures, and agree to abide by them. I agree to indemnify and hold harmless Columbia Access TV, its personnel and Stephens College from any and all liability arising from or in connection with the Program, or my acts or statements. I hereby grant Columbia Access TV permission to transmit the Program, retain a copy and retransmit the Program at its sole discretion.
This digital signature shall take the place of your written signature, but holds you to the above statements just as well.
